Holistic Case Management, LLC is a Medicaid funded program that provides daily assistance with personal care and case management to eligible members in a home setting. This program allows individuals who require assistance with activities of daily living to maintain their dignity and pursue typical life routines with individualized assistance from a family or non-family caregiver.Eligible recipients of the Monitored in Home Care program must be at least 16 years of age, have Medicaid insurance and require assistance with completing daily tasks from a Caregiver who lives in the same apartment or home.
